REQUIREMENTS FOR APPLICATION

  1. Applicant must be a graduate/holder of A.B./B.S. Degree (a 4- year or a 5-year course);
  2. Applicant must have taken the National Medical Admission Test (NMAT) and have obtained a score above the percentile cut-off set by CHED as recommended by the Technical Panel for Medical Education on a yearly basis. If the Applicant satisfies the above requirements, the Applicant may now:
    • Proceed to the Office of the Registrar for Application Requirements;
    • Pay the non-refundable Application and Processing Fees of USD $250.00
    • Please send your payment in a form of Manager’s Check/Cashier’s Check/ International Money Order addressed to UERMMMC, College of Medicine);
    • Submit the following documents not later than January 15, 2010together with the application form duly accomplished.
      1. Four (4) copies of the Official Transcript of Records duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin or legal residence, not “NOTED & SEEN” but with C-167-OCA-99 (2 original and 2 xerox copies)

      2. Diploma or Certificate of Graduation (1 original and 3 xerox copies)
      3. Seven (7) copies of Original Personal History Statement Form (PHS, Revised 1998), duly accomplished and signed by the applicant in English and in national alphabet, accompanied by personal seal, if any, original left and right hand prints and original photos;
      4. A Notarized Affidavit of Support and Proof of Adequate Financial Support, i.e., bank statement, to cover expenses for student’s accommodation and subsistence, as well as school dues and other incidental expense; (1 original and 3 xerox copies)
      5. Original and 3 xerox copies of Birth Certificate;
      6. Original and 3 xerox copies of Police Clearance from the applicant’s state of origin;
      7. Certificate of Good Moral Character from 2 former professors; (2 copies each)
      8. Result of National Medical Admission Test (NMAT); (Original and 3 xerox copies) and
      9. PREFFERED NMAT Percentile: 65

        April & December, 2009, April 2010

      10. Photo copy of Passport pages where name, photo, birth date and birth place appear, (3 xerox copies)

ADMISSION PROCEDURE

  1. UERMMMC receives application form together with the above documents required for a prospective foreign applicant;
  2. On the basis of temporary evaluation of the transcript of records, and other documents, UERMMMC issues a NOTICE OF ACCEPTANCE (NOA);
  3. Copy furnished the Commission on Higher Education (CHED) for the issuance of Certificate of Eligibility for Admission (CEA) into the Medicine course;
  4. CHED evaluates student records for issuance of CEA and
  5. UERMMMC endorses its recommendation to the Bureau of Immigration and Deportation (BID) for issuance of student visa.


N. B.

  1. Upon arrival of foreign student in the Philippines, please see the Registrar for information concerning issuance of Alien Certificate of Registration (ACR) and Certificate of Registration for Temporary Student (CRTS) by the Bureau of Immigration and Deportation (BID) and the enrollment procedure of UERMMMC.
  2. Visa Application Form, (FA Form No. 2) applicant’s personal appearance shall be required for filling out the form
  3. Medical Certificate (FA Form No. 11) including x-ray laboratory report and clearance certificate for the AIDS antibody
  4. Guarantee coverage of return fare and other incidental expenses
  5. Life Size Chest X-ray Film
  6. Eight (8) 2”x2” photos and
  7. Unexpired passport and USD $100.00 fee (U.S. Citizen, no fee)

Enrollment Procedure

  • Secure and fill-out the following forms at the Registrar’s Office;
    • Advising Slip (3 copies)
    • New Student Information Sheet (3 copies)
  • Have the forms signed by the Registrar or her authorized personnel or representative;
  • Proceed to MISD (2nd Floor, Admin. Bldg.), submit one copy of the accomplished New Information Sheet and Advising Slip to MISD authorized personnel for assessment and wait for your printed registration form for payment to the Cashier;
  • Pay the Matriculation Fee plus the USD $10,000.00 – Foreign Miscellaneous Fee (Payable upon Enrollment) and ID Fee at the Hospital’s Cashier located at the Ground Floor Hospital Building;
  • Proceed to the Infirmary Section (near Parking Area) of the Hospital for schedule of Physical Check-up;
  • Proceed to MISD (2nd Floor, Admin. Bldg.) for the schedule of ID picture taking;
  • Present your O. R. ID Fee and ID Payment slip.
  • Have your picture taken and wait for its issuance.

NOTE: COMPLETE UNIFORM IS REQUIRED BEFORE PICTURE TAKING.
